NYC Foodservice Establishment
Required Signs & Permits Check List

 Updated: 05/20/2012

THROUGHOUT:

  • EXIT SIGNS mounted over each required exit
     
  • EXIT DIRECTIONAL SIGNS mounted nearby, if Exit Sign is not in clear sight
     
  • NO SMOKING SIGNS post at all entrances as required including bathrooms, stairwells, on each floor, bulletin boards, and other prominent places DOWNLOAD HERE
     
  • WASH HANDS SIGNS (DOH) post at each hand wash stations and in all restrooms

 

FRONT OF HOUSE:

  • DEPARTMENT OF HEALTH PERMIT (DOH) post prominently for public view
     
  • HOW ARE WE DOING SIGN (DOH) post prominently for public view
     
  • LETTER GRADE or GRADE PENDING SIGN (DOH) post within 5 feet from entrance and 4-6 feet from ground or floor LEARN MORE
     
  • PERMITTED OCCUPANCY (DOB)
     
  • PLACE OF ASSEMBLY PERMIT (DOB), required for premises where 75 or more members of the public gather indoors or 200 or more gather outdoors
     
  • SMOKING PERMITTED, post if you have a sidewalk café, in up to 25% of sidewalk café seats.
     
  • NO SMOKING post at all entrances as required including bathrooms, stairwells, each floor, bulletin boards, and other prominent places. *No ashtrays are permitted in any indoor area DOWNLOAD HERE
     
  • SALE OF TOBACCO PRODUCTS TO MINORS (DCA or 311) post in a conspicuous place; can be obtained from teh Department of Consumer affairs or if you wish to post your own sign, the law requires that the sign be printed on a white card in red letters at least one half-inch in height.   • RESUSCITATION KIT SIGN (comes with kit) post, stating location of kit.
     
  • RESULTS OF LATEST HEALTH INSPECTION SIGN (DOH)
     
  • CHOKING POSTER, (DOH or Red Cross) post in all designated eating areas. LEARN MORE (page 28).
     
  • DANGER OF DRINKING WHILE PREGNANT SIGN (SLA), post in all designated drinking areas. LEARN More (page 27). 
     
  • NO ALCOHOL SERVED TO MINORS SIGN (SLA)
     
  • SIDEWALK CAFÉ PERMIT (DCA) post prominently in windows or doors that look onto the street
     
  • COMPLAINT SIGN (SIDEWALK CAFÉ) (DCA) post prominently in café area so that any member of the public will know where to write or call with problems regarding the cafe
     
  • LIQUOR LICENSE (SLA) post in frame in window near door; if no door, prominent place
     
     

 

BACK OF HOUSE

  • SCHEMATIC OF HOOD & DUCT SYSTEMS, FAN TERMINALS (from contractor) post in glass frame
     
  • CLEANING INSTRUCTIONS OF HOODS AND DUCTS (from contractor) post in glass frame
     
  • RECORD OF HOOD CLEANING (from contractor) post
     
  • FIRE EXTINGUISHERS’ RECORD OF ANNUAL INSPECTION (from contractor) tag each unit
     
  • SEMI-ANNUAL FORE SUPPRESSION SYSTEM INSPECTIONS (from contractor) tag or post copy
     
  • CERTIFICATE OF OCCUPANCY (Building Dept.) post copy
     
  • EVACUATION PLAN if located in high rise building, post
     
  • SPRINKLER INSPECTION, (from contractor) signed by maintenance contractor after each inspection
     
  • COMMERCIAL RECYCLING REGULATIONS LEARN MORE
     
  • FIRE ALARM TEST, (Combined form from Fire & Building Depts.) post record of monthly tests.
     
  • FIRE GUARD CERTIFICATE OF FITNESS (FDNY) Cabaret or Caterers, one Fire Guard required per Exit
     
  • FIRE DRILL CONDUCTOR CERTIFICATE OF FITNESS (FDNY) If more than one Fire Guard
     
  • FOOD ALLERGY INFORMATION DOWNLOAD HERE Must post in conspicuous locations where it will be visible to all food workers
     
  • WORKING CYCLIST SAFETY POSTER DOWNLOAD HERE- For businesses that employ commercial cyclists. LEARN MORE
